Introduction

The COPD Toolkit© was developed in response to demands from an increasing number of certified respiratory educators across Canada for practical resources related to establishing a chronic disease management program. The Saskatoon Health Region, the University of Saskatchewan and the Lung Association of Saskatchewan collaborated on this project in order to provide a practical blueprint for health care professionals to adapt and utilize in their own communities.

The Toolkit© may be used in whole or in part, depending on the needs of the patients in their community and the availability of resources. Users are requested to post their comments on each tool they use, including any modifications they have made to improve the tool or adapt it to their local program. Users are also invited to post their own COPD tools that they have developed in the COPD Toolkit© so that others can benefit from their experience and innovation.

The COPD Toolkit© is an open resource. The tools in the kit may be used and modified without copyright restrictions.
